Best mattress for back pain: how to choose
Hi, we’re John and Mimi. “I wake up with a sore back, which mattress should I get?” is one of the questions we hear most. There’s no single magic mattress, but there are a few things that matter a lot: how you sleep, how much you weigh, and whether your old bed is sagging. Here’s how to choose, and our picks at today’s prices: same factory models, lower price than other online retailers.
The short answer
- Most people with back pain do best on medium firm. Studies on mattresses and lower back pain have generally found that a medium firm bed works better for most people than a very hard one.
- Side sleepers usually want medium, with softer cushion at the shoulders. A zoned mattress does this on purpose.
- Stomach sleepers and heavier back sleepers often need firm, so the hips don’t sink.
- If your mattress has a dip or is past about 7 to 10 years, replacing it may help more than anything else. See signs it’s time.
What actually matters for a sore back
- A straight spine. Lying down, your spine should keep about the same shape it has when you stand up straight. Too soft and your hips sink like a hammock. Too hard and your shoulders and hips get pushed up instead of cushioned.
- Support under the hips. The hips are the heaviest part of your body, so that’s where a worn-out or too-soft mattress sags first.
- Pressure relief at the shoulders, especially if you sleep on your side.
- Easy to move on. If turning over takes effort, you may tense up at night. Bouncier foams and hybrids make it easier to change positions.
- The right firmness for your weight. Heavier people sink deeper, so the same mattress feels softer to them. Our mattress firmness guide explains the full scale.
Pick by how you sleep
| You sleep mostly on your… | Firmness to look for | Why |
|---|---|---|
| Back | Medium firm | Supports the lower back while giving a little cushion. Firm if you’re heavier. |
| Side | Medium, ideally zoned | Shoulders and hips need to sink a bit so the spine stays level. See our side sleeper guide. |
| Stomach | Firm | Keeps the hips from sinking and arching the lower back. See our firm mattress guide. |
| All of them (combination) | Medium to medium firm, responsive | A bouncier feel makes switching positions easier. |
Memory foam, hybrid or zoned?
- Memory foam molds to your body and relieves pressure points. Choose a medium firm one if you want support without feeling stuck.
- Hybrid (pocket coils under the foam) feels a bit more lifted and bouncy, sleeps cooler, and is easier to move around on. Many heavier sleepers like the extra push-back from the coils.
- Zoned mattresses are softer under the shoulders and firmer under the hips, which is exactly what a lot of side and back sleepers with sore backs need.

Tips that help as much as the mattress
- Give a new mattress a few weeks. Your body needs time to adjust after an old, sagging bed, and foam softens slightly as it breaks in.
- Back sleepers: a small pillow under the knees takes pressure off the lower back for many people. The zero gravity position on an adjustable base does the same thing with one button.
- Side sleepers: a pillow between the knees keeps the hips level, and a pillow tall enough to fill the space between your ear and shoulder keeps your neck straight.
- Stomach sleepers: use a thin pillow, or none, so your neck isn’t bent.
- Check what’s under the mattress. A sagging box spring or a frame with no center support can make a good mattress feel bad. Our adjustable base for back pain guide covers bases, lumbar lift and more.

Questions we hear a lot
What type of mattress is best for back pain?
For most people, a medium firm mattress that keeps the spine straight. Side sleepers usually do better on medium, ideally zoned, and stomach sleepers on firm. Memory foam, hybrid and zoned mattresses can all work if the firmness is right for you.
Is a firm mattress better for a bad back?
Not always. A very hard mattress can push up on your shoulders and hips instead of cushioning them. Most people with back pain do better on medium firm. Firm suits stomach sleepers and heavier back sleepers best.
Can a mattress cause back pain?
An old or sagging mattress can, because your hips sink and your spine curves overnight. If you wake up stiff but feel better after moving around, or sleep better somewhere else, your mattress may be the reason.
How long does it take to get used to a new mattress?
Usually a few weeks. Your body needs time to adjust after an old bed, and foam softens slightly as it breaks in.
Does an adjustable base help back pain?
Many people find that raising the knees and head a little, the zero gravity position, takes pressure off the lower back. Pair it with a mattress that bends, like memory foam or a pocket-coil hybrid. Questions? Text John at 805-617-6878.
